SUMMARY OF DEATH
Marcello Meadows, a 10-month-old boy from New Haven, Connecticut, died on June 28, 2023, from fentanyl, xylazine, and cocaine intoxication, ruled a homicide. His mother, Alexandra Polino, had a long history of opioid misuse and criminal justice involvement. Marcello was born in August 2022 with prenatal exposure to cocaine and opioids, diagnosed with Neonatal Opioid Withdrawal Syndrome and Failure to Thrive. DCF opened a case and created a safety plan, but multiple systemic failures occurred: the mother tested positive for fentanyl on multiple occasions yet the treatment provider inaccurately documented her as having successfully completed treatment, leading DCF to close its case on June 7, 2023, just three weeks before Marcello's death. Post-death investigation found drug paraphernalia containing fentanyl, cocaine, xylazine, and caffeine residue in the bedroom where the mother slept with the children. Polino was charged with manslaughter and two counts of risk of injury to a child. Marcello was the 11th young child to die from opioid ingestion in Connecticut since 2020.
